The university's rich history began in 1892 in Kirksville, Missouri, when Dr. Andrew Taylor Still established the American School of Osteopathy (ASO). Dr. Still's vision attracted an inaugural class of 21 students, graduating just two years later. Early on, the school solidified its foundation by hiring formally trained professionals like William Smith as its first anatomy professor, demonstrating a commitment to rigorous education from the outset. Over the years, ASO underwent several significant mergers and name changes, evolving to meet the growing demands of healthcare education.

ATSU's academic strengths are rooted in its pioneering osteopathic medicine program and have expanded significantly. The university demonstrated an early commitment to community health by establishing rural clinics, with the first opening in 1949, showcasing a dedication to serving underserved populations that continues today. By 1960, this program had grown to 10 clinics serving 43,000 patients. The institution further strengthened its research capabilities with a generous donation from the Rockefeller family, leading to the Timken-Burnett Research building.
